Obtaining Medical Records
 

All requests for medical records must be directed to Family Matters Counseling Center. Please email records request to familymatters@familymatterscounselingcenter.com.

 

A reasonable fee is included for providing these requested records as stated under the 22 Tex. Admin. Code § 165.2. The reasonable fee is a flat rate of $25 for pages less than 500 and $50 for pages over 500. These charges are included for labor, copying, and preparing the records. 

 

A copy of my medical records will be sent by mail within 15 business days after payment is received for copies ("business days" do not include weekends). Family Matters Counseling Center, LLC does not provide medical records by email, fax, or e-fax.
